Vitamin D levels cannot EVER become negative for any reason. It is probably just plain old '4' and not negative 4.
I don't know what else is going on with you or how bad you feel, so I wouldn't have any idea whether or not you would need to be hospitalized or not. But you DO need intense treatment.
That may mean getting a dose of up to ONE MILLION IU's in a very short period of time. I would not accept getting less than 600,000 IU's in the first month that you are being treated.
Otherwise, please be sure to read my page on Magnesium and Vitamin D. If you take these large doses of vitamin d that you need without taking magnesium along with it, you could have some VERY bad side effects.
